Our Primary Mental Care Check-In is a short, practical visit (15–30 minutes) designed for moments when something feels off and you need fast clarity. It functions just like a primary care “sick visit,” but for your mental and emotional well-being. You don’t need a diagnosis or ongoing therapy, just a concern you want evaluated by a trained clinician. This appointment helps you understand what is happening, what might be triggering the change, and what next steps are appropriate. You leave with answers, a plan, maybe a note for work or school, and the peace of handling it early.

All evaluations include standardized measures, clinical interpretation, and clear, no-jargon recommendations overseen by licensed psychologists.
Full Comprehensive Evaluation ($524.30)
Includes up to 4 standardized psychological tests to assess cognitive, academic, emotional, behavioral, and diagnostic concerns. Ideal for complex presentations, school accommodations, or diagnostic clarification. This may be needed when someone is struggling in multiple areas (school, work, emotions, focus, behavior) and a full picture is needed to understand what’s really going on and what supports will actually help.
Half Comprehensive Evaluation ($427.30)
Includes up to 2 standardized tests, providing focused assessment for targeted concerns while maintaining clinical depth and accuracy. This may be needed when there is one or two specific struggles and the goal is to get clear answers without doing a full evaluation.
ADHD Evaluation ($250.00)
Includes 1 standardized ADHD assessment, evaluating attention, executive functioning, and behavioral patterns across settings. This may be needed when documentation must meet gold-standard testing requirements for school, college, or workplace accommodations, meaning the results are formal, defensible, and accepted by institutions that require validated ADHD testing.
Autism Evaluation ($427.30)
Includes up to 4 standardized autism assessments, examining social communication, behavioral patterns, and developmental functioning. This may be needed when an initial autism diagnosis is being considered and a comprehensive evaluation is required, as autism assessments rely on multiple measures to accurately understand strengths, challenges, and support needs.
Diagnostic Evaluation Only ($195.90)
Includes 1 diagnostic measurement with clinical interpretation for diagnostic confirmation or treatment planning.
This may be needed when a diagnosis needs to be confirmed or re-certified for documentation purposes (such as school, services, benefits, or ongoing care), and one clear answer is needed rather than a full evaluation of multiple concerns.

Students are expected to manage heavy workloads, stay organized, focus for long periods, perform under pressure, and make decisions about their future—often all at once. Many students struggle not because they lack ability, but because their learning needs and strengths haven’t been clearly identified.
Perkins PsyCare Clinic provides college readiness testing focused on academic accommodations, learning direction, and career-aligned strengths. Using standardized, digital assessments, we evaluate how a student learns, focuses, manages demands, and performs under academic stress.

College Readiness Testing Fee: ($650) payment arraignments available.
College readiness testing is completed over two separate days, with approximately two hours of testing each day. This structure is intentional and important. Splitting testing across two days allows the brain to rest, reduces fatigue, and helps ensure results accurately reflect a student’s true abilities rather than exhaustion or test overload.
